Yourinformation.

Your name, contact information, and the infusion you would like to discuss, followed by a brief safety screening.

Alpha Vital Elite provides elective outpatient IV infusions, not emergency care. After your information we will ask brief yes-or-no questions about your current health and recent medical events, and Dr. Aronov reviews every answer before any infusion is given.

✦Questions
Brief yes-or-no questions about your current health and recent medical events, one at a time. There is no unsure option — if you are unsure, answer yes.
1Ends it
A single yes ends the screening. You will see why, and no appointment is scheduled. It is not a judgement about you — it means an elective infusion is not appropriate today.
✉Sent to the practice
Your details and your answers are sent to Dr. Aronov either way, whether you finish the questions or stop partway. Nothing is charged at any point.

Screening ended

Based on your answers, Alpha Vital Elite cannot provide an elective infusion at this time, and no appointment has been scheduled. Your answers have been sent to Dr. Aronov, who may follow up. If you think you answered by mistake, go back and change it. Questions: (347) 714-8660.

Nothing here rules out an infusion.

Passing this screening does not mean you are approved. Final eligibility requires physician review of your history, medications, allergies, laboratory information and requested infusion.

Dr. Violeta Aronov will contact you within 24 hours to ask any necessary follow-up questions and, if the infusion is appropriate, schedule your appointment. The complete medical form and patient attestation are reviewed and signed on-site on the day of your infusion. No screening or physician-review fee is charged.

Important: Eligibility form submissions are not reviewed immediately. Do not use this form for urgent or emergency medical concerns. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
