A physician will review the health information you provide and determine whether the IV support offered at Alpha Vital Elite is appropriate for you before an appointment can be scheduled. To request an appointment

Begin your eligibility review

Every appointment requires a physician eligibility review. A physician reviews the health information you provide — including your medical history, medications, allergies, and any available laboratory results — to determine whether the IV support offered at Alpha Vital Elite is appropriate for you. An appointment can be scheduled only if you are approved.

No fee The screening is a short questionnaire and there is no screening or physician review fee. If IV support is appropriate, Dr. Violeta Aronov contacts you directly to schedule your appointment. The complete medical form and patient attestation are reviewed and signed at the clinic on the day of your infusion. Completing the questionnaire does not guarantee approval. Refund Policy
