SECTION 01All payments are final
All payments made to Alpha Vital Elite IV are final and non-refundable. This includes any payment made for intravenous therapy. There is no charge for the safety screening or the physician review that precedes it, so no payment is collected before an infusion is approved and scheduled.
The only exception is described in Section 05 and applies when the practice cancels.
Alpha Vital Elite IV provides care by appointment only. Before an infusion may be scheduled, a physician reviews the health information you submit and determines whether the intravenous services offered by the practice are appropriate for you. Some patients may not be eligible. Please read this policy in full before making a payment.
SECTION 02The safety screening is free
There is no screening fee and no physician review fee. The safety screening is a short questionnaire, and the physician's review of your medical history, medications, allergies, and any laboratory results you provide is performed at no charge.
If IV support appears appropriate, Dr. Aronov will contact you directly to ask any follow-up questions and schedule your appointment. The complete medical form and patient attestation are reviewed and signed at the practice on the day of your infusion.
Payment is collected only for an infusion that has been approved and scheduled. Because nothing is charged for the screening or the review, there is nothing to refund at that stage.
SECTION 03Eligibility is a medical decision
Whether intravenous therapy is appropriate for you is determined by Dr. Violeta Aronov, D.O., using independent clinical judgment after reviewing the health information you provide. When appropriate, Dr. Aronov may also consult or coordinate with your primary care physician or treating specialist. That determination governs whether treatment may be provided by this practice.
Following that review, Dr. Aronov may approve the infusion you requested, recommend a different infusion, request additional information, defer treatment until you have spoken with another physician, or determine that intravenous therapy should not be provided. Expressing interest in a particular infusion does not guarantee that you will be eligible to receive it.
Any decision to modify, defer, or decline treatment is made in the interest of your safety and appropriate care. Completing the safety screening does not guarantee approval or create an entitlement to treatment.
SECTION 04Cancellations and missed appointments
If you need to reschedule your appointment, please call 347 · 714 · 8660 or email contact@precisionmedicaliv.com. We will make every reasonable effort to offer you another available appointment time.
Canceling, rescheduling, or missing an appointment does not entitle you to a refund. All amounts already paid remain non-refundable. At the practice's discretion, an amount already paid may be credited toward a rescheduled appointment.
SECTION 05If the practice cancels
If Alpha Vital Elite IV must cancel your appointment, we will make every reasonable effort to offer you an alternative appointment time. If none of the available times works for you, any amount paid toward that appointment will be refunded in full. This is the only circumstance in which a refund will be issued.
SECTION 06Completed and partial infusions
Once an infusion has begun, no full or partial refund will be issued. Each infusion is prepared specifically for you, cannot be reused or administered to another patient, and requires physician time and clinical resources committed to your care.
Dr. Aronov may stop the infusion at any time if, in that clinical judgment, continuing would be unsafe. You may also ask that the infusion be stopped. In either circumstance, the full charge remains due. Any decision to stop an infusion is governed solely by patient safety and clinical judgment, never by financial considerations.
SECTION 07Outcomes are not guaranteed
Infusion services offered by Alpha Vital Elite IV are supportive in nature and are intended to complement — not replace — appropriate primary or specialty medical care. They are not intended to diagnose, treat, cure, mitigate, prevent, or reverse any disease, medical condition, or inflammatory disorder and have not been evaluated by the FDA for those purposes.
Individual responses and outcomes vary, and no particular result can be promised or guaranteed. Nothing stated by the practice — whether on this website, during a consultation, or in any other communication — constitutes a promise of a specific outcome. Dissatisfaction with an outcome, including one that differs from what you hoped or expected, does not qualify for a refund.
SECTION 08Assumption of risk
All medical procedures carry risk, including intravenous therapy. Risks include but are not limited to discomfort, bruising or irritation at the infusion site, allergic or adverse reaction, and, rarely, more serious complications. These risks are discussed with you before treatment and you will be asked to give informed consent.
You are responsible for giving complete and accurate information about your medical history, medications, allergies and health conditions. Withholding or misrepresenting that information may make treatment unsafe, and treatment provided in reliance on incomplete information is at your own risk.
If you feel unwell following an infusion, contact the practice. If it is an emergency, call 911 or go to your nearest emergency department immediately. An adverse reaction does not, on its own, entitle you to a refund, and nothing in this policy limits any right you have under applicable law.
SECTION 09Payment disputes
If you believe a payment was charged in error, please contact the practice first. Most payment questions can be reviewed and resolved promptly, often with a single phone call, and contacting us directly is generally the fastest way to address the issue.
Initiating a dispute or chargeback with your card issuer for a service that was provided or a consultation that occurred does not alter the terms of this policy or, by itself, create a right to a refund. Nothing in this section limits any right you may have under applicable law.
SECTION 10Your acknowledgment
By making a payment for an infusion, you acknowledge that you have read, understood, and accepted this policy. You specifically understand that payments for intravenous therapy are non-refundable except as expressly provided in Section 05.
Completing the safety screening does not guarantee approval for intravenous therapy or create any entitlement to treatment. Eligibility is determined by Dr. Aronov based on independent clinical judgment after reviewing the health information you provide. That eligibility determination is final.
Individual responses vary, and no particular result or outcome is promised or guaranteed.
SECTION 11Changes to this policy
This policy may be updated. The version in force is the one published on this page on the date of your payment, and the effective date is shown at the top.
